Financial: Disability Tax Credit

When are you eligible?  

A qualified person must certify on this form that you have a prolonged impairment, and that the effects of the impairment are such that one of the following applies:  

  • You are blind, even with the use of corrective lenses or medication.  
  • You are markedly restricted in any of the following basic activities of daily living: walking, speaking, hearing, dressing, feeding, elimination (bowel or bladder functions), perceiving, thinking, and remembering  
  • You need, and must dedicate a certain amount of time specifically for, life-sustaining therapy.

If you receive Canada Pension Plan or Quebec Pension Plan disability benefits, workers’ compensation benefits, or other types of disability or insurance benefits, it does not necessarily mean you are eligible for the disability amount.  These programs have other purposes and different criteria, such as an individual’s inability to work.  

The Canada Customs and Revenue Agency must validate this certificate for you to be eligible for the disability amount.  

Visually impaired persons can get this form in Braille, large print or etext (computer diskette), or on audio cassette by visiting the site or by calling 1.800.267.1267 weekdays from 8:15 am. to 5:00 pm. (Eastern Time).
